Before participating in any class, outreach, tournament, transportation, volunteer service, or Foundation activity, every adult participant must complete this registration and liability waiver.

Liability Waiver & Assumption of Risk

Acknowledgment of Risk

I understand that participation in Brazilian Jiu-Jitsu, self-defense, fitness training, competitions, outreach events, transportation, volunteer service, and any other activities sponsored or conducted by Youth-Reach Costa Rica involves inherent risks, including sprains, fractures, dislocations, concussions, illness, permanent disability, property damage, and, in rare cases, death. I voluntarily choose to participate with full knowledge and acceptance of these risks.

Assumption of Risk

I voluntarily assume full responsibility for any injury, illness, loss, damage, or expense that may arise from my participation in Foundation activities.

Release and Waiver of Liability

To the fullest extent permitted under the laws of the Republic of Costa Rica, I release, waive, and hold harmless Youth-Reach Costa Rica, its Foundation, Board of Directors, officers, Foundation leaders, instructors, volunteers, representatives, facility owners, partner organizations, and affiliates from any and all claims, liabilities, demands, lawsuits, damages, costs, or expenses arising out of or relating to my participation, except where liability cannot legally be waived.

Medical Responsibility

I certify that I am physically capable of participating. I understand that I am responsible for my own medical insurance and medical expenses. In an emergency, I authorize Foundation leaders to obtain emergency medical treatment on my behalf if I am unable to provide consent.

Participant Code of Conduct

Respect, Safety & Conduct

I agree to treat every person with dignity, respect, and kindness; follow the instructions of Foundation leaders; help maintain a safe, welcoming, Christ-centered environment; and demonstrate good sportsmanship and self-control.

Alcohol and Drug Policy

Participants who arrive intoxicated or under the influence of alcohol or illegal drugs will not be permitted to participate. They may be immediately removed and may be suspended or permanently dismissed from the program. Our mission is to help individuals overcome addiction, not enable or perpetuate it. If you need help, please reach out to Foundation leaders.

Emotional Control and Safety

Reckless behavior, unnecessary fighting, aggression, threats, intimidation, bullying, harassment, or conduct that endangers others will not be tolerated. If I become emotionally overwhelmed or unable to conduct myself appropriately, I agree to remove myself from training until I have regained composure. If I fail to do so, Foundation leaders may remove me and impose discipline, including suspension or permanent dismissal.

Photography, Video & Social Media Authorization

I authorize Youth-Reach Costa Rica to photograph, video record, and otherwise capture my likeness, voice, and participation during Foundation activities. I understand that these photographs, videos, and recordings may be used without compensation for educational, ministry, promotional, fundraising, website, print, and social media purposes. I understand that once content is published online or through social media, Youth-Reach Costa Rica cannot control or guarantee future use or redistribution by third parties. By signing this agreement, I voluntarily grant this authorization as a required condition of participation.
